Indications/Uses
Peripheral & central neuropathic pain in adult. Adjunctive therapy in adults w/ partial seizures w/ or w/o secondary generalisation. Generalised anxiety disorder (GAD) in adults. Management of fibromyalgia.
Dosage/Direction for Use
Adult Neuropathic pain Initially 150 mg daily as 2 or 3 divided doses. May be increased to 300 mg daily after an interval of 3-7 days & if needed, to a max dose of 600 mg daily after an additional 7-day interval. Epilepsy Initially 150 mg daily given as 2 or 3 divided doses. May be increased to 300 mg/day after 1 wk. Max: 600 mg/day after an additional wk. GAD 150-600 mg daily given as 2 or 3 divided doses. May be started as 150 mg daily & increased to 300 mg/day after 1 wk. Following an additional wk, may be increased to 450 mg/day. Max dose: 600 mg daily after an additional wk. Fibromyalgia Usual dose range: 300-400 mg daily in 2 divided doses. Dosing should be begin at 75 mg bid (150 mg/day) & may be increased to 150 mg bid w/in 1 wk based on efficacy & tolerability. May be further increased to 225 mg bid. Max dose: 600 mg daily after an additional 1 wk. Patients w/ renal impairment CrCl ≥60 mL/min Starting dose: 150 mg/day as bid or tid. Max dose: 600 mg/day as bid or tid, ≥30 to <60 mL/min Starting dose: 75 mg/day as bid or tid. Max dose: 300 mg/day as bid or tid, ≥15 to <30 mL/min Starting dose: 25-50 mg/day once daily or bid. Max dose: 150 mg once daily or bid, <15 mL/min Starting dose: 25 mg/day. Max dose: 75 mg/day. Supplementary dose following haemodialysis Starting dose: 25 mg/day as a single dose. Max dose: 100 mg/day.
Administration
May be taken with or without food.
Special Precautions
Discontinue treatment if symptoms of angioedema occur. Hypersensitivity reactions. Diabetic patients. Dizziness, somnolence, loss of consciousness, confusion & mental impairment may occur. Vision-related effects. Renal failure. W/drawal of concomitant antiepileptic drugs. W/drawal symptoms. Elderly CV compromised patients. Central neuropathic pain due to spinal cord injury. Monitor patients for signs of suicidal ideation & behaviours & consider appropriate treatment; symptoms of misuse, abuse or dependence. Reduced lower GIT function. History of substance abuse. Encephalopathy. Minor or moderate influence on the ability to drive & use machines. Women of childbearing potential must use contraception. Pregnancy & lactation. Elderly >65 yr.
Adverse Reactions
Dizziness, somnolence, headache. Nasopharyngitis; increased appetite; euphoric mood, confusion, irritability, disorientation, insomnia, decreased libido; ataxia, abnormal coordination, tremor, dysarthria, amnesia, memory impairment, disturbance in attention, paraesthesia, hypoaesthesia, sedation, balance disorder, lethargy; blurred vision, diplopia; vertigo; vomiting, nausea, constipation, diarrhoea, flatulence, abdominal distension, dry mouth; muscle cramp, arthralgia, back pain, pain in limb, cervical spasm; erectile dysfunction; peripheral oedema, oedema, abnormal gait, fall, feeling drunk, abnormal feeling, fatigue; increased wt.
Drug Interactions
May potentiate the effects of ethanol & lorazepam. May impair cognitive & gross motor function w/ oxycodone.
